Incident or reportable near miss? Hire a professional external health & safety consultant to support your accident investigation and help prevent a recurrence.
When a workplace accident happens, the immediate priority is to make sure anyone who’s been injured receives the right assistance and that nobody else is put at risk. But once the situation’s under control, what happens next? For employers, investigating an incident properly is an important step towards preventing the same thing happening again.
It’s tempting to treat an accident as an unfortunate one-off, particularly if the injury seems minor. However, every incident deserves appropriate consideration. Something as simple as a trip, a minor cut or a piece of equipment failing could reveal a problem with working practices, training or the workplace itself.
An effective investigation isn’t about simply finding someone to blame. It’s about understanding what actually happened and why. That means looking beyond the immediate cause. If an employee slips on a wet floor, for example, it’s worth asking why the floor was wet, whether warning signs were used and whether cleaning arrangements were adequate.
Gathering information while events are still fresh can be helpful. Depending on the circumstances, this might involve speaking to witnesses, examining equipment, taking photographs and reviewing relevant risk assessments or procedures. Any findings should be recorded clearly, along with the action needed to address the problems identified.
Employers should also consider whether an incident needs to be reported under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR). Not every accident is reportable, so it’s important to understand which incidents meet the relevant criteria.
Perhaps most importantly, lessons need to be put into practice. Updating procedures, improving equipment, providing additional training or changing the layout of a work area can all help prevent a repeat occurrence.
